IT Equipment Reservation Form

Instructions and Guidelines for Use

  • Equipment may be reserved by School of Pharmacy students, staff, and faculty.

  • Equipment reservations must be submitted at least 2 business days before the requested date of use.

  • All equipment is reserved on a first come, first served basis. Advanced reservations are strongly recommended to ensure equipment availability.

  • All equipment must be picked up or returned between the hours of 8:00 AM and 5:00 PM in Beard 110 (Tech Core).

  • Equipment may only be used for School of Pharmacy business or activities.

  • No unauthorized software is allowed on School of Pharmacy computers.

  • Please inform the IT Department of any cancellations as soon as possible.
